BEFORE REGISTERING FOR THE MDO SUMMER PICNIC YOU MUST READ THE FOLLOWING DUTY OF CARE SAFETY PRECAUTIONS AND ACKNOWLEDGE THE COVID 19 WAIVER
A) THE MDO SUMMER PICNIC IS ONLY FOR MDO MEMBERS WHOSE MEMBERSHIP IS NOT EXPIRED
-if your membership is expired and you have not been able to pay your annual fee using any of the available options (Paypal, eTransfer, mail) we will accept cash on site
-attendance at the party is open to MDO members, friends and family
B) EACH MEMBER AND EACH GUEST MUST REGISTER INDIVIDUALLY AFTER READING THIS DOCUMENT IN FULL
C) MDO IS LOOKING FORWARD TO HAVING EVERYONE GATHER FOR AN OUTDOOR SUMMER PICNIC AT ANDREW HAYDON PARK UNDER THE GAZEBO! EACH MEMBER AND EACH GUEST WILL RECEIVE A PICNIC MEAL FROM SUBWAY CONTAINING A 6 INCH SUB, COOKIE AND BAG OF CHIPS. OTHER INDIVIDUALLY WRAPPED SNACKS, SOFT DRINKS AND WATER WILL BE AVAILABLE ON SITE AS WELL. PLEASE MAKE YOUR SANDWICH SELECTION WHEN REGISTERING FOR THE PICNIC USING THE REGISTER BUTTON BELOW. ALCOHOL IS NOT PERMITTED DURING OUR EVENT.
D) MDO IS BEING PROVIDED THE GAZEBO ON THE WEST SIDE OF THE PARK TO HOLD OUR PICNIC. ENTER THE PARK OFF CARLING AVENUE AT HOLLY ACRES ROAD AND TURN LEFT. CAPACITY UNDER THE GAZEBO AT PICNIC TABLES IS LIMITED HOWEVER BRING YOUR LAWN CHAIR OR BLANKET AND FEEL FREE TO SIT ANYWHERE CLOSE BY. WE WILL ALL PARTICIPATE IN A FUN GAME OF BINGO AND THERE WILL BE PRIZES AVAILABLE!
E) ON THE DAY OF THE PICNIC (JULY 19TH) IN ORDER TO ATTEND, EACH MEMBER AND EACH GUEST MUST ANSWER “NO” TO EACH OF THE NINE SCREENING QUESTIONS BELOW:
1. Are you currently experiencing one or more of the symptoms below that are new or worsening?
• Fever and/or chills
• Temperature of 37.8 degrees Celsius/100 degrees Fahrenheit or higher
• Sore throat
• Headache
• Cough or barking cough (croup) - not related to asthma, post-infectious reactive airways, COPD, or other known causes or conditions you already have
• Shortness of breath - not related to asthma or other known causes or conditions you already have
• Decrease or loss of smell or taste - not related to seasonal allergies, neurological disorders, or other known causes or conditions you already have
• For adults > 18 years or older - Unusual tiredness, lack of energy (not related to depression, insomnia, thyroid dysfunction, or other known causes or conditions you already have)
• Fatigue, lethargy, malaise and/or muscle aches/joint pain - if you received a COVID-19 vaccination in the last 48 hours and are only experiencing mild fatigue, muscle aches, and/or joint pain that only began after vaccination, select “No.”
• For children < 18 years - Nausea, vomiting and/or diarrhea not related to irritable bowel syndrome, anxiety, menstrual cramps, or other known causes or conditions you already have
Symptoms should not be chronic or related to other known causes or conditions. The symptoms listed above are the symptoms most commonly associated with COVID19. If you have these symptoms, you should isolate and seek rapid antigen testing.
2. In the last 14 days, have you travelled outside of Canada AND been advised to quarantine? • Yes • No
3. Has a doctor, health care provider, or public health unit told you that you should currently be isolating (staying at home)? This can be because of an outbreak or contact tracing. • Yes • No
4. In the last 5 days, have you been identified as a “close contact” of someone who currently has COVID-19? If public health has advised you that you do not need to self-isolate, select “No.” • Yes • No
5. In the last 5 days, have you tested positive on a rapid antigen test or a homebased self-testing kit? If you have since tested negative on a lab-based PCR test, select “No.” • Yes • No
6. In the last 5 days, have you received a COVID Alert exposure notification on your cell phone? If you have since tested negative on a lab-based PCR test, select "No." • Yes • No
7. In the last 14 days, has someone in your household (someone you live with) travelled outside of Canada AND been advised to quarantine? • Yes • No
8. In the last 5 days, has someone in your household (someone you live with) been identified as a ”close contact” of someone who currently has COVID-19 AND advised by a doctor, healthcare provider or public health unit to self-isolate? • Yes • No
9. Is anyone you live with currently experiencing any new COVID-19 symptoms and/or waiting for test results after experiencing symptoms? • Yes • No
F) AT THE PICNIC, ASK OTHERS IF THEY WOULD PREFER A 6FT DISTANCE BE MAINTAINED BETWEEN THEMSELVES OR INDIVIDUALS OUTSIDE OF THEIR HOUSEHOLD
G) UPON ARRIVAL DO NOT IMMEDIATELY GREET OTHERS WITH A HANDSHAKE OR HUG SINCE THIS MAY NOT BE WELCOMED
H) IT IS RECOMMENDED THAT MASKS BE WORN AT ALL TIMES EXCEPT WHILE EATING OR DRINKING HOWEVER IT IS NOT MANDATORY
